  8. Enter your last name capitalizing only the first letter (Key in your name as it appears on your license.)
  9. Enter your Social Security number - no dashes nor spaces
  10. Create a password
  11. Confirm your password
  12. Package code is: 3DQPA - check with founder
  13. Click on Logon to NoMoreForms
  14. Click Logon To NoMoreForms (On returning visits to the page, you just sign in under the "Returning Applicant" section using your same password)
