ALL CONTRACTING REQUIREMENTS PLEASE READ: download (205kb)

All contracts must be complete before sending them in for contracting. This includes state insurance license, proof of E&O, and voided check for EFT. Please list on the cover page the name and phone number of your recruiter.

SPECIAL NOTE

All agents please note before getting paid from insurance companies it is your responsibility to make sure your license is in place.
