Procedures for Filling out Contracts

1. Use the new Aviva contract. (61267) Aviva - 10/08
2. Send in all 5 pages of Global contract.
3. Make sure all questions are answered correctly on page 2 of contract.
4. Founders must sign annualization form.
5. Send in copy of license.
6. Send in copy of E & O or enrollment form.
7. Fill out Anti-Money Laundering sheet.
8. Fax transmittal to 765-676-7612 showing level and who they point to.
9. Fax Contract to 888-329-1329
